Mentor Expression of Interest

Please complete this form if you are interested in being a mentor in the ACCCN Mentorship Program.

Preference will be given to nurses who

• are ACCCN members (and have been involved in College activities)
• have at least 10 years experience in critical care
• are still active in critical care, whether employed or on a voluntary basis
• have held a senior role in any area of critical care (clinical, management, education, research)

This form will help us identify the best mentor/mentee match.
